When you request a guide, join an event list, submit optional context, or book a Clarity Call, Mind Body Worx may collect information you choose to provide, such as your name, email address, phone number, service interest, preferred format, appointment details, and message.

Do not use website forms to send detailed medical or mental-health information. The forms are not monitored for emergencies.

How information may be used

Service providers

HighLevel is intended to remain the system of record for contacts, forms, calendars, reminders, and workflows. Hosting, analytics, email, and messaging providers may also process limited information. The final policy must name the services actually used and describe their roles.

Campaign and usage information

The website may preserve UTM source, medium, campaign, content, and landing-page information to understand which outreach led to a request. The final analytics configuration, cookie behavior, and consent requirements must be reviewed before activation.

Marketing choices

Requesting a resource or service contact does not automatically grant marketing consent. Optional marketing consent is collected separately where applicable. Marketing recipients should be able to unsubscribe using the link provided in each message.

Retention, access, correction, and deletion

Security and sensitive information

No internet transmission can be guaranteed completely secure. Mind Body Worx should limit access to submitted information and avoid sending sensitive coaching intake content to an AI service without explicit consent, documented retention rules, and human review.

Children

The intended audience is adults. The final policy should confirm any age restrictions and the process for handling information submitted by a minor.

Updates to this policy

The approved policy should display an effective date and explain how material changes will be communicated.
